SWEEPS® Study Club Workshop: Fotona Laser in Endodontics

Fotona’s minimally invasive SSP and SWEEPS® endodontic laser treatments are fast, safe and effective solutions for cleaning and disinfecting root canal systems.
This Endodontic workshop is designed for general dentists as well as for endodontic specialists to take a step forward towards modern laser-assisted endodontics. It involves step-by-step theoretical presentations of procedures, followed by hands-on on extracted teeth with learning objectives focused on root canal irrigation.
Each participant should bring one extracted tooth with the access cavity prepared (removed pulp chamber roof only). Teeth should be kept in 5% formaldehyde water solution or 0.9% Saline solution.
SWEEPS® technology represents a breakthrough method for 3D cleaning and disinfection of the root canal system. Er:YAG laser technology is used to activate the commonly used irrigants in endodontics with the laser tip kept in the access cavity only while simultaneously irrigating the whole root canal system at once. By using synchronized pairs of ultra-short pulses, an accelerated collapse of laser-induced bubbles is achieved, leading to enhanced shockwave emission even inside the narrowest root canals. SWEEPS® endodontic laser treatment successfully addresses a major disadvantage of classical chemo-mechanical treatment procedures which is the inability to completely clean, debride and disinfect anatomically complex root canal systems.

Laser Activated Irrigation - From Basic to Complex Endodontic Cases
Laser Activated Irrigation (LAI) is an accepted, evidence-based method of activated irrigation in endodontics, even though it is still not deservedly recognized. Better understanding of the challenges in contemporary chemo-mechanical processing of the root canals, as well as laser physics, dental laser device properties and latest breakthroughs in laser activated irrigation methods may 'shine a light' on everyday endodontic clinical practices. The lecture will cover the basic principles of dental laser application, with the emphasis on endodontics and development of LAI.
Various clinical cases will be presented, from the relatively simple to more complex endodontic cases.
The lecture will address the application of LAI in endodontic therapy of internal and external root resorptions, especially external cervical resorption cases.

Calcifications and broken files. Management of challenging cases in endodontics using SWEEPS technology
Some of the most challenging situations in endodontic treatment are calcified canals and cases of broken file retrieval. However, both problems have different causes, they are very time consuming and require highly skilled operators. Fortunately, the development of new technologies allows us to solve endodontic problems easier and faster. One of the newest technologies available on the market is the Er:YAG laser SWEEPS mode. During the lecture you will see protocols which will redefine your strategy for the most challenging cases. You will get tips on how to use less files, spend less time and at the same time achieve successful endodontic treatments.

Endodontic Treatment planning with CBCT-Technology

Aims
The purpose of this presentation is to provide the advantages and benefits of CBCT in the diagnosis of endodontic disease and to create consequently an optimal treatment plan for the patient. The introduction of cone beam computed tomography (CBCT) for every day practice improved the diagnosis and therapy significantly in general dentistry and especially in the endodontic treatment. The presence and location of periapical changes or root resorptions are identified more accurately and earlier through the CBCT. The additional information leads to a more accurate diagnosis and adequate therapy, thus results in a better treatment planning and prognosis estimation for the patient
Learning Objectives
The main advantages of CBCT are pointed out, especially for resorptive lesions or pain cases with negative radiographic findings. In addition, the limitations of CBCT are discussed and illustrated with clinical cases. In this interactive presentation standard and special cases will be presented and everyone of the audience can select the appropriate treatment plan via a voting system. Then the different results are presented and discussed together for each case.

(14h30 - 16h00) NiTi kinematics: rotation Vs. reciprocation

Aims
The aim of the workshop is the profound understanding of kinematics of the movements most commonly used during root canal shaping with NiTi tapered instruments. The physical basics of interaction of the file blades with the dentinal walls in different anatomical situations will lead to the full comprehension on the ideal action that clinicians should use to maximize the efficiency and safety of both movements with different sequences in common clinical situations.

Learning the ideal motion to apply in different clinical situations to rotary and reciprocating files; understanding the differences in kinematics between the two movements; improve safety in the use of NiTi instrumentation.
(16h15 - 17h45) Advantages of Reciprocating motion in endodontic retreatments

Aims
Starting from the kinematics and physical basics of reciprocating movement, the aim of this workshop will be to understand the advantages of reciprocating movement when dealing with retreatments, removal of hard obturations materials, management of ledges and perforations, improving diagnostic efficiency and possibility to manage these types of challenging clinical scenarios. The possibility to act as a digger cutting with the tip of the file is a property strictly linked with a file in reciprocation and this property will give great advantages in the mentioned clinical cases.
Learning Objectives
Learning to remove any type of old obturation materials without the use of solvents using a single file in reciprocation; improve safety during retreatment cases; manage ledges directly with a mechanical file in reciprocation; improve diagnostic and therapeutic efficiency in perforation cases.

To achieve a safe and predictable endodontic treatment protocol using Protaper Ultimate (PTUL) as instrumentation system together with newest endodontic motor developed by Dentsply Sirona X-Smart Pro+ (XSP+).
Furthermore, irrigant activation with Smart Lite Pro Endoactivator (SLP-EA) and single-cone technique obturation with AhPlus Bioceramic Sealer will be used.

Aims
Management of extreme anatomical challenges in Endodontics.The design and biological objectives of root canal treatment manipulation are well described in the contemporary art of Endodontics. Achieving these objectives in straight canals is considered a simple and straightforward procedure with all instrumentation systems available today. The problems of biomechanical instrumentation and obturation of the root canal systems come when the internal anatomy of human teeth is rendered severely curved or even bifurcated and anastomotic. In such teeth, the accepted basic endodontic techniques and instrumentation protocols may be challenging to follow.The aim of this lecture is to describe the instruments and the clinical skills needed to manage complicated cases.
